Request for Transcript
Please follow the following steps to request a transcript:
- Please click here to download the request for the transcript form.
- Print and completely fill out the form, with your signature and date.
- Scan or take a picture of the signed transcript request.
- Send an email to transcript@pitc.edu and attach the form. Do not hand over the form to anyone.
- Your transcript will be mailed to you. For any questions, please send an email to transcript@pitc.edu.
- You are required to pay by credit or debit card at pitc.edu and attach the payment to the transcript request email.
